Vaginismus as a hidden cause in infertility

Abstract

Vaginismus, an involuntary contraction of the pelvic muscle significantly interfere vaginal penetration because of the pain. This problem can persist for a long time, bring distress to the couple and lead to infertility. Vaginismus often underdiagnosed and neglected and patients sometimes do not know what to do or how to consult. There were three cases of vaginismus that came to our clinic. Two cases were completed and resulted in pregnancy while one case is still ongoing. Case 1 30 years old women who has been married for 3 years and never had a successful intercourse. On physical examination, grade 1 vaginismus was concluded and she agreed to use dilator. The use of dilators lasted for 11 months gradually started from the smallest for 4 months, medium for 4 months and the largest for 3 month. After 11 months intercourse was able to be done completely although the patient still complained of pain and pregnancy occurred 1 month later. She is now a mother of 2 years son. The second case, woman 33 years old who has been married for 4 years. She had history of PCOS and her husband had mild abnormal sperm parameter. The couple planned to begin pregnancy program soon and she was referred for Botox assisted dilation. The procedure went smoothly. They underwent insemination 2 month later and resulted in pregnancy. Management of vaginismus must consider biopsychosocial and decision for treatment should be tailored to patient’s need.
