Audit of operative vaginal delivery and infection reduction in operative vaginal delivery a cross-site re-audit

Abstract

Operative vaginal delivery (OVD) is a critical obstetric procedure used when intervention is necessary to ensure the safety of both the mother and the infant. While OVD can be life-saving, it carries certain risks, including increased maternal and neonatal complications, particularly infections. These infections can result in prolonged hospital stays, additional healthcare costs, and adverse outcomes for both the mother and baby. Therefore, reducing infection rates during OVD is of paramount importance to improving patient outcomes

This re-audit aimed to assess compliance with the standards for operative vaginal delivery (OVD) and infection reduction across three sites of Manchester Foundation Trust: ORC, Wythenshawe, and NMGH, focusing on compliance with established guidelines and standards for infection prevention as delineated by the Royal College of Obstetricians and Gynaecologists and local protocols. Previous studies have indicated that factors such as the adequacy of consent documentation, use of prophylactic antibiotics, and adherence to sterility protocols play a crucial role in reducing the incidence of infections during and following operative vaginal deliveries. 

By conducting a cross-site re-audit, this study endeavored to evaluate changes in practice and compliance since the last audit, identified areas needing improvement, and promoted strategies for enhancing overall outcomes in OVD. Data analysis revealed improvements in documentation of consent and use of sterile personal protective equipment but highlighted persistent challenges in areas like paired cord blood sampling and complete documentation of procedures. Compliance with antibiotic administration also showed variation across sites. 

Based on these findings, the audit team proposed an action plan to improve compliance, including educational initiatives, the creation of smart phrases for documentation, and improvements in aseptic techniques. The expected outcomes include better patient recovery, reduced infection rates, and enhanced overall care in instrumental deliveries.
