Glycocalyx investigation and therapeutic application in maternal sepsis
Abstract
Among pregnant, laboring, post-abortive and postpartum women worldwide, 7.8% suffer from infections, of which 0.3% are at risk of developing sepsis, especially in the postpartum period, of which 50% of cases lead to death in hospital. Unfortunately, the predictive value of obstetric infection guidelines for identifying maternal and puerperal sepsis is only 20.3%. And in terms of treatment, only 15.4% of women with puerperal sepsis receive antimicrobials according to national guidelines, and 2/3 of women receive broad-spectrum antibiotics.
Hyperreactivity to infection that occurs as a result of impaired immune system function and can be clinically evidenced is called sepsis. During sepsis, the immune system releases inflammatory mediators, which damage the carbohydrate layer covering the gel-like inner part of the endothelial cell (EC) membrane, called the glycocalyx (GCX). The intervillous space of the fetal placenta is lined with syncytiotrophoblast GCX – additional source of transmembrane protein syndecan-1 (SDC-1), which is released into the circulation during pregnancy as an adaptive response to increased maternal blood volume through endothelial remodeling. However, the degree of SDC-1 elevation may be a useful predictor of sepsis-related complications and mortality.
Studying GCX structures in maternal sepsis is a relevant research topic, as its degradation reflects endothelial dysfunction and may serve as a biomarker for sepsis severity and vascular barrier compromise.
Biography
Anastasia Muntean graduated from the Medical and Pharmaceutical College "Raisa Pacalo", worked in the emergency department of the Institute of Emergency Medicine and in the first line during the Covid-19 pandemic at the Municipal Clinical Hospital "Gheorghe Paladi", completed an internship at the STROKE department of the Institute of Neurology and Neurosurgery "Diomid Gherman" and in the department of Liver, gallbladder and pancreas surgery of the Republican Clinical Hospital "Timofei Moșneaga". A young researcher in the field of biochemistry, she successfully published her first scientific work "A comprehensive review of glycocalyx investigation and therapeutic applications in sepsis and septic shock" in the Romanian Journal of Laboratory Medicine.
