Can the bone dimensions of the pelvis be related to the genesis of pelvic organ prolapses? A systematic review of observational studies for risk assessment and prognosis

Abstract

Pelvic Organ Prolapse (POP) is the protusion of pelvic viscera through the pelvic floor and vaginal walls – responsible for the sensation of “ball in the vagina”, as well as the presence of symptoms such as urinary and fecal incontinence, bleeding, discharge and vaginal infections. As it directly affects women’s quality of life, its genesis has been studied and has presented several branches of causality: advanced age, menopause, previous hysterectomy and, for some, altered bone diameters


Objective: To investigate the role played by the bone dimensions of the pelvis in relation to the genesis of pelvic organ prolapses through publications from the last fifteen years.


Methods: This is a systematic review of observational studies for risk assessment and prognosis through a virtual bibliographic survey of scientific articles published in digital journals, in PubMed, BVS and ScienceDirect databases.


Results: A wider anterior pelvic area and a larger interspinous diameter were characterized as possible causes for pelvic organ prolapses. Most of the studies have diversified bone measurements, in which the other dimensions weren’t statistically significant.


Conclusion: The studies evaluated in this review suggest a new measurement of the pelvic floor related to women with prolapse, with a larger anterior area, largely influenced by the interspinous diameter, which leads to an increased load on the pelvic floor. However, even so, there is an urgent need for further studies to corroborate our findings.

Biography

Magda Patricia Furlanetto is graduated in Physiotherapy and Biology. She is a specialist in Physiotherapy in Women's Health (COFFITO) and holds a specialization in Molecular Biology, a master's degree in Genetic and Molecular Diagnosis, a doctorate in Health Sciences, and is a Post-doctoral researcher in the PPG of Rehabilitation Sciences. She also holds a postgraduate degree in Integrated Clinical Osteopathy. She taught Neuromuscular Anatomy in the Medicine course for 17 years. Currently, she is a professor at the University of Vale do Rio dos Sinos (Unisinos) in subjects related to Pelvic Physiotherapy, Physiotherapy in Oncology, and Curricular Internships. She is the founder of the Academic League of Pelvic and Oncological Physiotherapy at UniRitter and the Study Group in Pelvic and Oncological Physiotherapy at the University of Passo Fundo. She is a Lato sensu professor of Pelvic Dynamics, Pelvic Floor Assessment and Dystopias, Geriatrics, and Pelvic Dysfunctions at Faculdade Inspirar. Physiotherapist responsible for the Physiotherapy service at the Espaço Saúde Beleza Clinic. Has experience in teaching, academic extension, research and patient care related to Human Anatomy and Pelvic Physiotherapy, as well as private practice. Member of ABRAFISM (Brazilian Association of Physiotherapy in Women's Health) and FSR - Fascia Society Research. Co-founder of Cyclica - Education and Health.
