A case of torsion in an otherwise-normal ovary in a 22-years old nulliparous women: Utilization of diagnostic laparoscopy for the accurate diagnosis

Abstract

Ovarian torsion is the fifth most common gynecological emergency with a reported prevalence of 2.7% in all cases of acute abdominal pain. efinitive diagnosis is sometimes difficult to establish because of clinical symptoms that are similar to those other abdominal diseases.We report 22-year-old woman nullipara with acute lower right abdominal pain for 14 hours before coming to an emergency room. On physical examination,the abdomen soft but tender over the right iliac fossa and rebound tenderness was present.Ultrasound radiological examination results showed that the right ovary is grossly enlarged in size (volume- 98.4cc) with hyperechoic central stoma. Multiple tiny follicles seen within stroma. On colour doppler twisted pedicle was noted. Mild collection is also seen in pouch of doglus. During laparoscopy, we found a blackish-red colour cystic mass of about 12 × 11 cm that experiencing torsion. Then salpingo- ophoorectomy was done.

Biography

Shain Fariya Shetu, MBBS, FCPS (Obstetrics and Gynaecology), is an assistant professor in the department of obstetrics and gynaecology at Popular Medical College, Dhaka, where she has been serving since June 2025. Prior to this, she worked as a registrar in the same department from June 2021 to May 2025, after completing extensive postgraduate clinical training at Dhaka Medical College Hospital and Bangabandhu Sheikh Mujib Medical University. She completed her MBBS from Bangladesh Medical College Hospital and earned her FCPS in obstetrics and gynaecology from the Bangladesh College of Physicians and Surgeons in 2021. She also passed MRCOG entry exam from the Royal College of Obstetricians and Gynaecologists, UK, and holds additional certifications in diabetology and medical education.She is actively involved in undergraduate teaching, clinical training and research. She has published 13 original articles, reviews, and case reports in national and international peer-reviewed journals. She is a Fellow and Life Member of the Bangladesh College of Physicians and Surgeons, a life member of OGSB, a member of RH STEP and the Bangladesh Menopause Society, and serves as a gynecologist with the NGO “Do Better for Children,” reflecting her commitment to both academic excellence and community-based women’s health care.
