Predictable, preventable, and pugnacious approach

Abstract

Taj Mahal is known as a monument of India but has an underlying sorrow of maternal mortality. Despite rapid strides in maternity care, Postpartum hemorrhage[PPH] remains a global threat. Even though the Incidence is 1-10% of deliveries, difference in mortality rates exists between low and high income countries namely 25% and 5-9 % respectively. The need of the hour is to change the paradigm and prevent mothers dying in childbirth. The golden hour is the precious time to judiciously use the available facilities to combat PPH.

Our institution is a well equipped tertiary care centre. Prophylactic measures are universally followed right from AN care upto parturition. We report the data from January, 2023 to September 2025 [33 months]. Out of 3616 deliveries 38 patients had PPH and the incidence of PPH is 1.05%. Primi-73.7%, rest were multi. Between 25-35 years of age 71.1%, twin gestation seen in 18.4%, Spontaneous conception in 57.9% and ART conception in 42.1%. Gestational age 57.9% were>37 weeks, 10.5% were 26-30 weeks. The risk factors are Preeclampsia 15.8%, PAS / Placenta Praevia 10.4 % and Abruption 13.2%, Systemic Co morbidities in 53.3%. Uterine atony -89.5% and traumatic 7.9%. Mode of delivery was LSCS in 71.1%. 2 patients had Caesarean hysterectomy for Placenta Acreta Spectrum [PAS], surgical intervention in 34.2% and Uterine artery embolization in 07.9%.


Complete range of Uterotonics, Tranexamic acid, blood component therapy, Uterine artery embolization, round a clock anaetheists and multidisciplinary team are our pillars to tackle the problem effectively. Being the referral centre, we come across the high risk population and salvage the preterm neonates as well, in level III NICU. By adapting timely strategies we could help mothers tide over the crisis. Follow up of these patients are done meticulously to look for any sequalae such as lactation issues, anemia, depression, etc. 


WHO road map to combat PPH is an important milestone towards this burning problem. As an obstetrician we stand together to achieve this goal.

M Banumathy is a well-known obstetrician and gynaecologist associated with Sri Ramakrishna Hospital. She has 42 years of experience in obstetrics and gynaecology and has worked as an expert in different cities in India. She has contributed to handling numerous complex medical cases in several hospitals. She is known for her attention to accurate diagnosis and treating patients empathetically. The speciality interests of her are Obs/Gyane, Gynaecology Laparoscopic surgery, High-Risk Delivery. She is also an active member of the Indian Medical Association (IMA). She did her MBBS and diploma in obstetrics and gynecology from Madras Medical College. She also completed her DNB obstetrics and gynecology from National Board of Examination. She has also participated in research work and various workshops under the gynaecology department and published many papers.
